Overview

PruittHealth Hospice is our family-focused approach to end-of-life care that helps patients live their final days well cared for, pain-free, and surrounded by their loved ones. Hospice care does not hasten or postpone death, caring for the person rather than the disease, with a focus on quality rather than length of life. It is always family-centered, involving the patient and family in the decision-making process. PruittHealth delivers hospice care 24 hours a day, 7 days a week in the setting tailored to each patient's individual needs. Hospice is not a place; it is a specialized service approach to end-of-life care.

 

We also offer palliative care, which is a 24/7 care management service to help patients with acute or chronic illnesses get back to a higher level of comfort. Palliative care focuses on symptom management and stress relief. The goal is to reduce a patient's suffering, provide ongoing treatment, and promote the best quality of life for those with serious illnesses.

 

Through our hospice and palliative care services, both patients and families will find comfort, support, personal and spiritual peace, and an appreciation for the fullness of life.

Admissions

All patients are admitted and treated without regard to race, color, creed, age, handicap, national origin, or payment classification. We strive to meet any special cultural or psychosocial requirements. For admission, we will need:​

 

Upon admission, please bring the following with you:​

  • Patient history of hospice services
  • Record of in-home care currently received
  • Copy of the patient's Medicare Beneficiary Identifier Card (MBI)
  • Copy of any advanced directives, DNR orders, health care Power of Attorney, etc.
  • Current medications and any medical equipment in the home
  • Contacts for family members that are allowed to receive updates
