PruittHealth - Parkwood is an Intermediate Care Facility that serves Individuals with Intellectual Disabilities (ICF/IID) across the state of Georgia. Our specially trained staff supports clients with daily needs, provides engaging activities and opportunities to build essential skills, and empowers each person to achieve greater independence.

 

We partner closely with families and guardians to create personalized plans of care that support growth, well-being, and quality of life while fostering a true sense of community and belonging.

Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) provide comprehensive, individualized care and support designed to help individuals build skills, strengthen independence, and enhance their quality of life.

 

At PruittHealth - Parkwood, we bring that support to life through engaging daily activities, targeted skill development, and compassionate caregivers who encourage each client to grow and thrive. We also keep parents and guardians actively involved, from helping shape individualized plans of care to staying connected through phone calls, letters, facility visits, and therapeutic home visits.

Residential Programming at PruittHealth - Parkwood

We encourage clients to participate in community and recreational activities, and our programming is designed to include a wide range of stimulation for each client. Our interdisciplinary care team develops an individualized programming plan that accounts for clients' strengths, interests, and limitations, with the help of the parents, guardians, or representatives.

24-Hour Nursing Services

Licensed nurses provide around-the-clock medical care, health monitoring, and emergency support to ensure safety and well-being.

Support and Training for Activities of Daily Living (ADLs)

We help clients build independence with everyday activities such as bathing, dressing, eating, personal hygiene, and household tasks.

Behavior Management

Specialized support is designed to address challenging behaviors and promote positive behavioral outcomes. Services may include behavior assessments, development of support plans, staff training, and implementation of strategies that enhance safety and communication.

Communication Development

We support verbal, nonverbal, and assistive communication skills to improve self-expression and understanding.

Dietetic Programming

Registered dietitians develop individualized meal plans, provide nutrition education, and address specialized dietary needs.

Medication Management

We ensure medications are administered safely while collaborating with prescribing physicians. Services may include prescription coordination, documentation, and training for self-administration of medication.

Sensorimotor Therapeutic Exercise Program

Our expert sensorimotor team provides therapeutic exercises and activities, motor skills training, and dietary and feeding support. These services enhance functional abilities, promote independence, and improve overall quality of life.

Community Integration Training

Clients participate in recreational and educational activities in the community that promote social and life skills, such as shopping, dining, or attending the movies.

Pre-Vocational Training

We offer training programs that help individuals develop the work-related skills, habits, and behaviors necessary for future independence, which can include task completion, following directions, time management, workplace communication, and responsibility.

Skill Acquisition Programs

Our programs provide individualized training that focuses on developing new skills and enhancing existing abilities in areas such as social interaction, community living, problem-solving, and independence.

School-age clients continue to attend Valdosta City Schools, supporting their ongoing education and socialization with classmates.

To meet the DSM-5 criteria for an Intellectual Disability diagnosis, all three of the following conditions must be present:

  • Deficits in intellectual functioning - deficits in “reasoning, problem solving, planning, abstract thinking, judgment, academic learning, and learning from experience” as confirmed by clinical evaluation and individualized standard IQ testing
  • Deficits in adaptive functioning and in the ability to adapt to environment and the ability to perform activities of daily living
  • The onset of these deficits during childhood
Common ICF Diagnoses

At PruittHealth - Parkwood, we recognize that every individual has unique strengths, abilities, and support needs. Our interdisciplinary team focuses on providing person-centered services that promote independence, dignity, and the highest possible quality of life.

Individuals with Intellectual and Developmental Disabilities (I/DD) may have diagnoses such as:

  • Down Syndrome
  • Cerebral Palsy
  • Fetal Alcohol Spectrum Disorders (FASD)
  • Autism Spectrum Disorder (Level 3)
  • Genetic Syndromes
  • Chromosomal Disorders
  • Other Developmental Disabilities
When Residential Care May Not Fit

Our admissions team and interdisciplinary staff review each referral individually to ensure prospective clients can be safely supported within our residential environment while maintaining the health, safety, and quality of life of all clients who call Parkwood home.

PruittHealth - Parkwood does not provide on-site psychiatric services, psychiatric crisis stabilization, or BCBA therapy. Individuals who require these specialized services may be better served in programs that offer intensive behavioral health or psychiatric treatment.

At this time, our community may not be the appropriate placement for individuals who:

  • Are experiencing an acute behavioral health crisis or currently require crisis stabilization services
  • Exhibit aggressive, assaultive, or violent behaviors that pose a significant safety risk to themselves, other clients, staff, or the community
  • Require frequent emergency psychiatric intervention or hospitalization
  • Are experiencing active psychosis, including severe hallucinations, delusions, or significant impairment in reality-based functioning
  • Have severe mental health conditions that require intensive psychiatric treatment, close clinical monitoring, or specialized behavioral health services beyond the scope of our residential program
  • Require ongoing one-to-one behavioral intervention services provided by a Board Certified Behavior Analyst (BCBA) or similar intensive behavioral support programs

Our Admissions Process

At PruittHealth - Parkwood, our goal is to make the admissions process as seamless and supportive as possible for clients and their families. Each client receives a thorough evaluation and personalized transition plan, helping them begin their journey at our community with confidence and support.

You'll begin with a screening call with our admissions team to discuss the client’s needs, eligibility, and potential fit for our residential program.
If eligible, families complete the PruittHealth - Parkwood application and submit supporting documents for review by our interdisciplinary care team (IDT).
    • Medical records
    • Individualized Education Program (IEP)
    • Psychological evaluation
    • Copy of Medicaid card
    • Social Security income verification
    • Guardianship paperwork
    • Other relevant records as requested
The client and family will visit PruittHealth - Parkwood to meet with our team and tour the campus. Members of the IDT will assess the client’s needs, strengths, and abilities while families explore whether Parkwood is the right fit for their loved one.
Once evaluations and documentation are complete, we submit the required materials to the Georgia Medical Care Foundation for placement approval.
    •  DMA-6
    • Physical examination and laboratory results completed within 30 days prior to admission
    • Comprehensive psychological evaluation completed within one year of admission
    • Social History
    • Admitting Developmental Care Plan
After approval, our team works with the client and family to schedule move-in and support a smooth transition to the Parkwood community. We also connect families with community resources, health care providers, and other services that can support the transition.
