PruittHealth @ Home is a full-service agency that allows patients to reside in the comfort of their own residence throughout treatment after illness or injury. Once patients have returned home, our home health nurses and support team monitor their well-being, as well as recommend lifestyle options, in order to maintain optimum health and wellness. Patients find restoration in the comfort of their homes a truly revitalizing experience, and PruittHealth @ Home strives to make each interaction as comfortable and unobtrusive as possible.

Admissions

All patients are admitted and treated without regard to race, color, creed, age, handicap, national origin, or payment classification. We strive to meet any special cultural or psychosocial requirements. For admission, we will need:​

 

Upon admission, please bring the following with you:​

  • Patient history of hospice services
  • Record of in-home care currently received
  • Copy of the patient's Medicare Beneficiary Identifier Card (MBI)
  • Copy of any advanced directives, DNR orders, health care Power of Attorney, etc.
  • Current medications and any medical equipment in the home
  • Contacts for family members that are allowed to receive updates
