PruittHealth Home First
PruittHealth Home First is a program designed to help eligible patients receive care in their home. Our program is dedicated to ensuring your safety and improving your overall health. We offer adult day health, emergency response, home meal delivery, support services, and much more in order to meet and exceed your individual health care needs and expectations.
Program requirements:
- 65 years old or older and/or disabled
- Receiving Medicaid and SSI
- Medical conditions such as diabetes, high blood pressure, lung disease, arthritis, Alzheimer’s disease, dementia, heart disease, or stroke that require monitoring and regular doctor’s care
Services:
- Adult day health
- Create and implement an individualized care plan
- Education on the disease process, treatment goals and self-care management
- Regularly review personalized Health Plan with member and primary care physician
- Management of conditions in home to prevent rehospitalization, ER visit, and unplanned physician's visit
- Bathing, dressing, toileting, hygiene, oral care and medication reminders
- Basic home activities – meal preparation and cleanup, light housekeeping, laundry, etc.
- Transportation to and from the doctor or other appointments and errands
- Companionship, respite care
- Home safety clearance and Emergency Response Monitoring System
