PruittHealth Care Management offers attentive, personalized service coordination for confident, customized care. PruittHealth Care Management starts with Patient Placement to help guide patients and families through the process. Our Transition Support Program has procedures for achieving rehabilitation goals and avoiding rehospitalization. Home First is our Care Management program in the home.Once placed into one of our Healthcare Centers or receiving services from our Family of Providers – Hospice, Home Health, or Infusion Pharmacy, managed care members receive individualized case management services through our team of nurses, all Certified Care Managers. The over-arching goal is to ensure a seamless, safe, and clinically appropriate pathway through the maze of health benefits and various levels of care offered by PruittHealth. To that end, we provide services 7 days a week to ensure that weekends are not an obstacle to accessing care within PruittHealth. We measure our success with metrics that focus on timeliness and clinical accuracy.
